Forms for Master's Students

Please refer to our dates and deadlines information for important Graduate School deadlines. In order to receive your degree, you will need to apply for graduation by the appropriate deadline even if you do not plan to attend the ceremony.

Your department and advisory committee are responsible for determining the manual of style for writing a research paper, thesis, or creative project; however, the Graduate School determines the proper formatting and submission procedures for the product. Make sure your project follows the formatting and submission requirements for a creative project, research paper, or thesis and submit your project correctly.

Forms for Doctoral Students

Please refer to the dates and deadlines section for important Graduate School deadlines. In order to receive your degree, you will need to apply for graduation by the deadline, even if you do not plan to attend the ceremony.

Your department and advisory committee are responsible for determining the manual of style for writing a dissertation; however, the Graduate School determines the proper formatting and submission procedures for the product. Make sure your project follows the formatting and submission requirements for a dissertation and submit your project correctly.

Students should not register for DISS 799 (the dissertation course) until they have received notification that the Graduate School has approved their admission to candidacy. Please speak to your advisor about registration options the term you plan to propose.

ProQuest / UMI and NSF

Instructions and links for submitting the ProQuest / UMI Publishing Agreement and the NSF Survey of Earned Doctorates will be individually emailed to eligible graduates by the Graduate School Graduation Coordinator once all applicable criteria have been met by the student.
