Request Types

Work Request - routine maintenance in academic, administrative, sport, and athletic facilities
Non-Emergency Room Repair - request maintenance in residence facilities
Key Request - key request or to change a core in a non-residence hall facility
Project Request Form - for renovation of a space or requesting new carpet, drapes, or tile.
Change in Assignment and Designation of Space - request to change room use (ex: classroom to office) or who is occupying the space (ex: Biology to Nursing).

Request for work in a Housing and Residence Life or University Apartment building, are submitted through Housing and Residence Life.

Equipment Move or Borrowing Requests
All requests for borrowing or moving Ball State equipment are handled through Purchasing Services. To file an equipment request online, log into SciQuest and complete a Equipment Request (B24 form) or a Move Request (B450 form).
