The following are requirements for students to participate in the laboratory classrooms at the Mitchell Early Childhood and Family Center. The Mitchell Center is a licensed, accredited and Paths To Quality Level 4 childcare center. Students who do not provide the approved, necessary documents will not be able to participate in the laboratory classrooms.

Detailed Information
- Fingerprinting Form for Background Check: Students are required to be fingerprinted through the state of Indiana.
- Use the website https://in.ibtfingerprint.com/ to make an appointment.
- For ECYF 265 students, enter service code Child Care Center Volunteer: 24Y7HV
- For ECYF 275 students, enter service code Child Care Center Volunteer: 24Y7HV
- For ECYF 465 students, enter service code Child Care Center Employee: 24Y7GB
- Students who send their results to the wrong location will have to repeat the process again. Fingerprinting results are accessible to our licensing agents for 3 years. If you have been fingerprinted for a childcare home or childcare center in the state of Indiana within the last 3 years, your records may still be in the system and you will not need to be fingerprinted again. You will need your fingerprinting receipt for our files. If you do not have this, you will need to be fingerprinted again. Once you have the fingerprinting form for background check, submit the form for documentation. Keep the original for your records. You will need the transaction code to complete the consent form on I-LEAD.

- Consent Form on I-LEAD: Once you have the Fingerprinting receipt for the Background check, you are able to complete the consent form on I-LEAD for the licensing agent to review your records. The consent form allows our state licensing agent to run the background check and clear you for participation in the classroom with children. To receive an invitation, email Jennifer Young your legal first and last name, date of birth, and email address. Once you receive the invitation, you will have 72 hours to complete the Consent to Release Information form on I-LEAD (Indiana Licensing Education and Access Depot).
- Tuberculin Skin Test:
Provide current documentation of a “0 mm” (negative) TB test. You may
choose any facility to complete the TB test. Students may obtain TB
tests at the BSU Health Center for $10, 765/285-8431, an appointment may
be required. Documentation is required within 30 days of the beginning of the semester. Results are valid for one year. Documentation to submit: TB test results card or Adult health exam with TB test results indicated

- Drug Screen: The Indiana Code 12-17.2-5-3.5 requires that each childcare center shall maintain and make available drug test results which do not show a presence of illegal controlled substances of any employee or volunteer caring for children prior to employment or volunteering. - The drug test must be dated within 60 days prior to participation.
- A negative result is required.
- A dilute negative result is not accepted by our licensing agent (this can happen when excessive water is consumed prior to providing a urine sample).
- Drug test results shall be reviewed by a nationally certified Medical Review Officer using positive cut-offs established by the U.S. Department of Transportation. Drug test results must include contact information for the Medical Review Officer and signature for all positive results. Negative results do not need a signature of a Medical Review Officer.
You may choose any facility to complete the drug screen. In Muncie, Cover-Tek, 765-896-8594, is located at 926 W. Main St. Muncie, IN 47305. They take walk-ins for drug screen. Cost for the drug screen is $55. Take a photo ID and a piece of identification with your signature. Show them thisform. Identify the company name as the Mitchell Early Childhood and Family Center. Ask to have the results emailed to you or plan to go back and pick up your results. It may be possible to complete the drug screen at the same time as the fingerprinting appointment. - Health Exam: A physical examination verification form is required with a physician or nurse signature. You may choose any facility to complete the physical examination. BSU Students may obtain free physical at the BSU Health Center, 765/285-8431, an appointment is required. Use the Adult Health Exam form. Documentation is required within 30 days of the beginning of the semester.
