If you would like contraception from the Health Center and
have never used anything in the past, please start by reviewing our page on
contraception and the CDC website.
Women take contraception for reasons in addition to
preventing pregnancy. Please discuss with a healthcare provider at the Health Center if you are having severe menstrual pain,
irregular periods, heavy menstrual bleeding, or severe PMS symptoms.
Your first visit for contraception will be a virtual visit with a healthcare provider in order to review birth control options, benefits and risks.
Afterwards, you will then be scheduled for an in-person appointment with a healthcare provider. Some questions will be asked
regarding your health history, your family history, smoking history and sexual
history. You will be given three
months of medication and asked to follow up with a visit after that time for a
weight, blood pressure check and medication refill.
If you are wanting the Health Center to continue
contraception that another medical provider has been managing, then please
click the link to download the Release of Information form. Complete the form and either send it directly
to your medical provider or fax to the Health Center at 765-285-1103. We will then fax it to your medical provider
so that we can obtain your medical records.
Please call us to make sure that we have received your records within
one week.
We do follow current ASCCP (American Society for Colposcopy
and Cervical Pathology) guidelines which
indicate a first liquid based pap smear, which is a cervical cancer screening
test, should be performed at age 21.
If you cannot get an appointment before your contraceptive
is going to run out, please speak with a nurse at the Health Center to discuss your options.
